REMOTE MONITORING OF BLOOD GLUCOSE LEVELS

Remote monitoring of blood glucose levels can be used to obtain information about a diabetic patient’s sugar levels, symptoms, diet, and medication adherence on a daily basis.  Long term, monitoring can lead to changes in a patient’s lifestyle and behavior and may be able to prevent serious complications like blindness, amputations, chronic kidney disease, and stroke.

Flexcare Health Solutions uses SmartMeter® technology to enable transmission of individual self-monitored blood glucose values to a certified diabetes educator and pharmacist who reviews the data and notifies the patient and providers that action is needed. Remote monitoring also includes a 15-30 minute monthly remote call or Skype with a certified diabetes educator to review the patient’s blood glucose log and make recommendations as needed.
