We provide coaching, training, and management services for people living with or at risk for diabetes. Our highly committed certified diabetes educators (CDE’s) provide in-person and remote training. Our small group and one-on-one in-person classes are located in our office in Washington DC or in the Flexcare Pharmacy classroom.
We specialize in: Adults with Type 2 Diabetes, Pre-Diabetes, and Gestational Diabetes.
REMOTE MONITORING OF BLOOD GLUCOSE LEVELS
Remote monitoring of blood glucose levels can be used to obtain information about a diabetic patient’s sugar levels, symptoms, diet, and medication adherence on a daily basis. Long term, monitoring can lead to changes in a patient’s lifestyle and behavior and may be able to prevent serious complications like blindness, amputations, chronic kidney disease, and stroke.
Flexcare Health Solutions uses advanced technology to enable transmission of individual self-monitored blood glucose values to a certified diabetes educator and pharmacist who reviews the data and notifies the patient and providers that action is needed. Remote monitoring also includes a 15-30 minute monthly remote call or Skype with a certified diabetes educator to review the patient’s blood glucose log and make recommendations as needed.

Our evidence based, patient driven program is accredited by the AADE (American Association of Diabetes Educators. We offer both group and one-to-one classes. Our certified diabetes educators develop individual action plans and provided continued support and education through a series of six classes. At this time our classes are cash-only and are scheduled in advance.
